I’ve been planning to write a guide on using Google Docs to create surveys. Thanks to my friend, Bob Ladd, I don’t have to reinvent the wheels. By reading a post on his Instructional Media blog, I came across a blog post from the Center for  Instructional Technology at Duke University, where they show instructors how to use Google Forms to create surveys for their courses. The step-by-step screenshots and instructions in this post are very easy to follow.
